Electronic Annual SCM Inspection FAQs

  • All annual inspections must be completed through the electronic process on the City website.  www.concordnc.gov/SCM-Inspections
  • Inspections using the old form will not be accepted.
  • You must click the correct dot on the map for each inspection.  Blue Dots require an Annual Inspection.  Orange dots require an As-Built Inspection.
  • The inspection must be completed between March 15 and May 15 of each year.  Inspections submitted before March 15 will need to be resubmitted, with updated pictures.
  • If you don't see the SCM you're trying to inspect, please email freemanh@concordnc.gov and we will add the point.
  • A North Carolina licensed Professional Engineer must complete the Annual Inspections.
  • A minimum of 4 photos are required.

Periodic SCM Inspections

Periodic inspections should be completed using the forms below, and kept on site until requested by the City.  Please do not send monthly reports to the City, every month.  An audit will be completed annually, where a month from the previous year will be requested.
